What data we process and why

We process the data entered in the inquiry form: full name, phone number, email address (if provided), pick-up and destination locations, preferred date and notes, which may include information about the patient’s health.

We process this data solely to respond to the inquiry, prepare a quote and organise the transport.

Legal basis

The legal basis is taking steps at the request of the data subject prior to entering into a contract (Art. 6(1)(b) GDPR) and, for health data, explicit consent (Art. 9(2)(a) GDPR). Consent can be withdrawn at any time without affecting the lawfulness of processing before its withdrawal.

Recipients

The form content is delivered to our mailbox through the external form service Web3Forms, acting as a data processor. Data may also be entrusted to hosting and email service providers.

Retention period

We keep inquiry data for as long as necessary to handle it and no longer than 12 months, unless the transport takes place - in that case, for the period required by law.

Your rights

You have the right to access, rectify and erase your data, to restrict processing, to data portability and to object. You also have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO) in Poland.

Providing data is voluntary but necessary to respond to the inquiry.
